Request for Extension of Course Work Completion
Basic Degree students in theology can use this form to request an extension of course work completion.
Study Carrel Application Form
Kelly Library Carrels, located on the third floor of the John M. Kelly Library, are available for the use of Advanced Degree students. Preference is given to doctoral students in the dissertation phase of their studies. Carrels are assigned for one academic year at a time (from September 1 to August 31) and students may reapply twice, for a maximum of three years of use. Applications are due by the second Friday of September each year.
Carrels are funded through a significant donation from the Basilian Fathers of the University of St Michaelās College. Carrel use is administered as a bursary.

Course Audit Application Form for Restricted Group of Course Auditors

Graduation Application
Graduate students in theology who have fulfilled their program requirements must apply to graduate. The application serves to alert the Student Services Officer to review an applicantās academic record for the purpose of granting a degree/diploma/certificate.
Students who have questions regarding program requirements may contact their Faculty Advisor or the Director of Basic or Advanced Degree Programs before applying.
The deadline for submission of the graduation application, in time for the annual graduation ceremonies in November, is usually the first Friday in September each year. (Please contact the Student Services Officer for the exact date.)
After verifying an applicantās academic record, the Student Services Officer will reply to the applicant in late September confirming eligibility to convocate.
